No insurance? No problem. Call us to find out about our self-paid and out-of-network rates.

Current in-network insurance plans accepted:

• Blue Cross Blue Shield PPO (Blue Choice, Anthem)

• Cigna and Cigna EAP

• Curalinc EAP

• Humana

• New Directions EAP

• Tricare / Humana Military

• United Health / Optum and Optum EAP

If your insurance is not listed above: you may claim your out-of-network benefits, which insurance may still reimburse but at a slightly lower rate than in-network benefits. Superbills are another option, which you would submit to your insurance for partial reimbursement. Ask your therapist about these options.

Payment:

We accept cash, checks, and all major credit cards as forms of payment. Payment by check should be made to the order of “Your Family Wellness Village LLC.” Payment is made at the time of the session.

Cancellation Policy:

If you are unable to attend a session, please make sure you notify us at least 24 hours beforehand. Otherwise, a late cancellation fee of $75 will be charged.
